BEFORE YOU REGISTER
NATC Training & Competency Evaluation Program
The cost of the eighty-three (83) hours class is $345.00.
Cost of the class must be paid in full by the start of clinical's. NATC accepts cash, personal checks, money orders, VISA, Master Card, American Express, Discover, or any combination thereof.

The fee for the class is non-refundable*. Trainees who cancel less than 7 days prior to a call may not be reconsidered for enrollment in a future class. Dates for the start of the classes are subject to change and are not guaranteed.

Trainees are expected to be on time a all times. Ten or more minutes late at the start of the class and clinical's will be counted as an absence.** This includes the start of class, return from all breaks and during clinical's. Two absences will result in expulsion for the class. A trainee who is expelled for attendance may have ONE opportunity to be reassigned to another class for a $50 reassignment fee. NO GUARANTEE of reassignment exists under ANY circumstances.

All returned checks will be assessed a $30.00 fee, which must be paid before the class begins.

From time to time photographs may be taken of trainees in action in the classroom and skills lab. Trainee authorize the use of any said photo in any future marketing material for NATC.

By initialing the registration form you acknowledge that you fully understand and agree to comply with all of these and all other rules, policies and procedures of the NATC Training and Competency Evaluation Program.

*NATC reserves the right to assess extenuating circumstances (i.e. death in immediate family) and reassign the trainee to another class should such a circumstance arise after classes have begun but NO GUARANTEE of reassignment exists under any ANY circumstances.

**Prior approval granted by the Program Coordinator for any tardiness or absences will not count against the trainee.

In order to qualify to work with a vulnerable population such as the elderly, children and people with developmental disabilities one must not have been charged with, convicted of, or plead guilty to any of the following. Per the Ohio revised code:
